For all appointments and inquiries, call (516) 616-5500 during normal business hours. For after hours, please use our answering message service.

We are open Monday - Friday (except Holidays) between 9:00am to 5:00pm. As a Specialist clinic, patients may need a referral from their general practitioner, family physician or other doctor to be eligible for coverage from their insurance carrier /Medicare.

When you come for your appointment remember to bring the following:

• Medicare, Insurance, or Veterans Affairs card.
• Referral letter from your GP, family physician or other doctor.
• Have your health insurance information.
• Related copies of operation records, medical records, x-rays, MRIs, CT scans, etc. from prior doctor visits.
• If you have had surgery, please bring a copy of your operation report.

To cancel an appointment

To cancel an appointment, call our office during business hours. Please cancel at least 1 day ahead so that your appointment time can be allocated to another patient who is seeking treatment.

We realize your time is valuable and we make every effort to run on time. As you can imagine, emergencies may occur that cause scheduling delays beyond our control. We apologize in advance if we keep you waiting.

Insurance Information

Most insurance plans are accepted.

Make sure to check with your insurance company to see if you are covered for weight loss surgery when booking your appointment.
